How the Solution Works

Where do we get the data to determine eligibility?
At Open City Labs, we offer multiple ways to seamlessly integrate data into our system to support eligibility checks. At its core, Navigator 360° is a standalone application that provides a simple and intuitive platform for entering data directly into the system.
However, we recognize the challenges of fragmented data and are committed to reducing silos, not adding to them. Open City Labs is the first care coordination application built with open standards in mind. One of our key strengths is our deep expertise in data standards and integrations. By leveraging open standards, we can seamlessly integrate with and extract data from most Electronic Health Records (EHRs) and other systems that support Fast Healthcare Interoperability Resources (FHIR) standards. Additionally, because our platform is built on these standards, we can also push data back into other systems.
We understand that the care coordination ecosystem includes a variety of systems, many of which may not yet be FHIR-capable. That’s why we’re committed to exploring different integration options to connect with Referral Management Systems, Case Management Systems, and other essential platforms to ensure a truly interoperable solution.
