For example, approximately 25% of people eligible for SNAP (Supplemental Nutrition Assistance Program) and 50% of those eligible for WIC (Women, Infants, and Children Program) do not participate. SNAP enrollment is associated with a reduction in annual healthcare costs of $1,400-$4,100 per person and WIC is shown to reduce the likelihood of preterm births by 48%.

Why does this happen?

The two primary barriers are lack of awareness and difficulty in applying. Many people are unaware of their eligibility or find the application processes overwhelming and time-consuming.

How the Solution Works

Open City Labs harnesses AI and Machine Learning to map eligibility requirements for 227 government programs (e.g., SNAP, WIC) across 35 government agencies and process PDF form data for 63 programs. This allows information entered in one application or pulled from a Health Information Exchange (HIE) to be automatically reused when applying for multiple programs.
By leveraging HIE data and automating form completion, our software:
  • Auto-fills up to 95% of fields in applications like WIC, similar to a university "common application."
  • Reduces SNAP application rejections by 47%, as demonstrated in a randomized control trial funded by the AARP Foundation.

With SMART-on-FHIR integration from the Social Care Network, Open City Labs’ Navigator360 software streamlines government benefits enrollment by enabling:

Where do we get the data to determine eligibility?

At Open City Labs, we offer multiple ways to seamlessly integrate data into our system to support eligibility checks. At its core, Navigator 360° is a standalone application that provides a simple and intuitive platform for entering data directly into the system. However, we recognize the challenges of fragmented data and are committed to reducing silos, not adding to them. Open City Labs is the first care coordination application built with open standards in mind. One of our key strengths is our deep expertise in data standards and integrations. By leveraging open standards, we can seamlessly integrate with and extract data from most Electronic Health Records (EHRs) and other systems that support Fast Healthcare Interoperability Resources (FHIR) standards. Additionally, because our platform is built on these standards, we can also push data back into other systems. We understand that the care coordination ecosystem includes a variety of systems, many of which may not yet be FHIR-capable. That’s why we’re committed to exploring different integration options to connect with Referral Management Systems, Case Management Systems, and other essential platforms to ensure a truly interoperable solution.
